HOW TO SUBMIT THE PRIOR TO ARRIVAL FORM

 

Step 1: Close registration for camp and collect the CODES General Release Waivers and Medical Form from each student that has registered. Ensure that all pertinent information has been completed accurately (we cannot accept forms that are incomplete or completed in languages other than English).

Step 2: Using the information on the collected forms, answer the questions on the Prior to Arrival form below. Please be as thorough and detailed as possible. We will reach out to you with any questions or needs for additional information that arise.

Step 3: Receive your Cabin and Trail Group rosters from us via email. Complete these by assigning a Cabin and Trail Group to each student that will attend camp and resubmitting both rosters to us.
